Mezuzah and Tefillin Checking

How to prepare your mezuzah for checking:
1. Carefully remove the mezuah from the doorway. The scroll should not be removed from the case.
2. Complete the form below and submit.
3. Print the completed form receipt and include it with your shipment.
4. Pack your mezuzot and the completed form receipt with some paper or bubble wrap. Ship via USPS priority or another trackable courier.

1. Leave the Tefillin inside its pouch.
2. Complete the form below and submit.
3. Print the completed form receipt and include it with your shipment.
4. Pack the tefillin with care. Ship via USPS priority or another trackable courier.
